CVS Health is seeking a Senior Epic Analyst for Healthy Planet & Care Coordination to design, build, and optimize Epic solutions that enhance population health management and care transitions across care settings. You will mentor analysts, partner with clinical leadership, and drive agile delivery, testing, and governance.

Qualifications

Bachelor's degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent experience).
4+ years of Epic experience, with substantial hands-on work in Healthy Planet and/or care coordination/care management modules.
Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules required or strongly preferred.
Demonstrated experience designing workflows for population health, care management, or value-based care programs.
Strong understanding of transitions-of-care processes and cross-continuum care delivery.

Responsibilities

Design, build, and optimize Healthy Planet registries, care plans, care teams, and health maintenance topics.
Configure and optimize risk stratification tools, care gaps, and outreach workflows within Healthy Planet.
Lead testing and validation of Healthy Planet builds to ensure accuracy of registry logic and reporting outputs.
