Job summary

CVS Health is seeking a Senior Epic Analyst for Healthy Planet & Care Coordination to design, build, and optimize Epic solutions supporting population health management and care transitions. You will mentor analysts and partner closely with clinical leadership to translate strategy into scalable Epic configurations.

Qualifications

• Bachelor's degree or equivalent experience in health informatics, information systems, nursing, or related field.
• Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules preferred.
• 4+ years of Epic experience with Healthy Planet and/or care coordination/care management modules.
• Experience designing workflows for population health, care management, or value-based care programs.
• Knowledge of transitions-of-care processes and cross-continuum care delivery.

Responsibilities

• Lead design, build, and optimization of Epic Healthy Planet solutions for population health management.
• Design care coordination workflows and optimize interoperability (Care Everywhere) across settings.
• Partner with stakeholders to translate program goals into technical requirements and timelines.
• Mentor junior analysts on Healthy Planet build standards, testing, and troubleshooting.
• Maintain documentation, SOPs, and backlogs; contribute to governance and release activities.
