Job summary

Pacific Health Group is seeking the ECM Senior Lead Care Coordinator – Onboarding Specialist to set the standard of care across the Enhanced Care Management department. This senior SME will lead onboarding, field-based mentorship, and ongoing coaching for new Lead Care Coordinators, bridging the gap to full field autonomy.

You will apply CalAIM expertise, oversee complex care coordination, and partner with leadership to refine workflows and documentation.

Qualifications

• Residency: Must reside in the hiring county.
• Senior Experience: 3–5+ years of direct ECM/CalAIM/medi-cal or complex community-based case management experience.
• Demonstrated Expertise: Proven track record in clinical preceptorship, staff onboarding, field coaching, or senior leadership in case management.
• Travel & Licensing: Valid CA driver's license, active auto insurance, reliable vehicle, ability to travel 10–30% locally within the county.
• Technical Infrastructure: Reliable computer for first 21 days (stipend provided) and high-speed internet (200+ Mbps down / 25+ Mbps up).
• Pre-Employment Screening: Background check (MVR) and Testlify skills assessment.

Responsibilities

• Lead end-to-end onboarding for new Lead Care Coordinators, ensuring mastery of CalAIM, population health frameworks, and PHG standards.
• Facilitate advanced instruction on assessments, care planning, motivational interviewing, trauma-informed care, and crisis de-escalation.
• Drive technical proficiency across TheraPlatform, Salesmate, Monday.com, RingCentral, and health plan portals.
• Conduct field ride-alongs and home-visit observations, modeling outreach, field safety, rapport-building, and community-based care.
• Guide staff through high-acuity cases, modeling problem-solving during crises and challenging member dynamics.
• Provide real-time feedback in field and remote settings to build confidence and autonomy.
• Train and audit on billable vs non-billable activity, notes, and regulatory timelines.
• Identify quality trends and develop retraining to maintain audit readiness; partner with ECM Leadership, QA, and L&D.
