Job summary

Wayfinder Family Services seeks an experienced Lead Care Manager to coordinate care for the Enhanced Care Management program in Stockton, CA. The role covers direct service delivery, care team collaboration, and advocacy to maximize client access to needed services.

Responsibilities include developing care plans, coordinating with healthcare providers, and supporting clients across office, telehealth, and in-field settings. The position emphasizes trauma-informed care and system navigation.

Qualifications

• Bachelor’s degree from an accredited university in a related field.
• At least three years of relevant work experience.
• Multilingual capabilities are preferred.
• Experience with EMR/EHRs/Care Management software is preferred.
• Long-time community knowledge a plus.

Responsibilities

• Oversees the development and implementation of the Individual Care Plan/Health Action Plan.
• Offers services where the client lives, seeks care, or finds most accessible (office, telehealth, or field).
• Connects clients to other social services and needed supports.
• Coordinate/advocate on behalf of the client with health care professionals (e.g., PCP).
• Utilizes motivational interviewing, harm reduction techniques, and trauma-informed care.
• Works with hospital staff on Transitional Care Planning.
• Conducts outreach and engagement to link clients to the ECM program (phone, mail, field visits).
• Accompanies clients to office visits as needed.
