Job summary

Fresno Mission seeks a Lead Care Manager to coordinate Enhanced Care Management for high-need beneficiaries, assess patient needs, and provide resources and coordination for healthy outcomes.

You will engage ECM-eligible members, oversee service delivery, and ensure care plans align with managed care guidelines and hospital discharge planning. A mature Christian faith and alignment with our mission are required.

Qualifications

• AA or higher in Drug and Alcohol, Social Work, Human Services or related field.

Responsibilities

• Coordinate with member’s service providers to ensure seamless care and avoid duplication of services.
• Actively engage ECM-eligible members and oversee ECM services and care plan implementation.
• Connect ECM members to social services and supports, including transportation.
• Advocate for ECM members with healthcare professionals and coordinate with hospital discharge planning.
• Maintain HIPAA compliance and document gaps in care plans for multidisciplinary teams.

Education
