Job summary

Eisner Health seeks a Lead Care Manager for the Enhanced Care Management (ECM) program, blending remote and on-site work. Expect 2–3 days onsite weekly as part of the hybrid arrangement.

The Lead Care Manager connects care team members with patients, coordinating plans to meet medical and psychosocial needs and reduce preventable hospital use. Requires bilingual abilities and relevant health experience.

Qualifications

• Bachelor degree required.
• Two years of experience in community health or social services setting required.
• English and Spanish fluency required.
• Proficiency in Microsoft Office Suite products.
• Ability to travel locally to Eisner Health sites and communities where ECM patients live.
• Must work in an interdisciplinary team setting.
• Experience with Electronic Health Records preferred.

Responsibilities

• Engages ECM patients and facilitates care management where patients live or seek care.
• Develops patient-centered Care Plans based on health needs and preferences.
• Ensures Care Plan is implemented and updated at least quarterly.
• Educates patients on self-management and involves caregivers when possible.
• Supports behavior change using motivational interviewing and trauma-informed care.
• Monitors treatment adherence and coordinates with ECM team.
• Initiates case conferences with providers and ECM Consultants.
• Connects patients to social services such as housing or transportation as needed.
• Coordinates discharge plans with hospital staff and transitional care.
• Accompanies patients to office visits as needed and per guidelines.
• Maintains contact schedule with enrolled patients and at least one in-person encounter per month.
• Documents encounters in EHR with proper billing codes.
• Tracks outreach attempts in ECM/spreadsheets and i2i group.
• Supports monthly ECM reports for each Health Plan.
• Performs other duties as assigned.
