Job summary

HeartShare Wellness LTD in New York is seeking a Sr Day Support Professional to oversee staff actions and implement person-centered services. The role ensures the wellbeing of individuals, their goals, and outcomes, with daily community and program activities.

Qualifications

• High school or GED required; bachelor’s degree from four-year college or equivalent preferred.
• Valid NYC Driver’s License.
• Minimum 2 years in the field with supervision and paperwork.
• Minimum 2 years with individuals with developmental disabilities.
• Prior staff supervision experience strongly preferred.

Responsibilities

• Oversee DSP implementation of individuals’ plans, addressing goals and outcomes.
• Promote choice and self-determination to support individuals’ control over their lives.
• Ensure DSPs support and teach based on services and outcomes; engage in skills development and socialization.
• Promote community inclusion and foster friendships with neighbors, coworkers, and peers.
• Transport individuals using agency vehicles or by public transportation as required.
• Plan monthly schedules for community outings and in-house activities; accompany individuals in the community.
