Job summary

HeartShare in New York, NY is hiring a Sr Day Support Professional to oversee DSP plan implementation and community activities. You will promote self-determination, coordinate services, and support a person-centered approach for each individual’s goals and well-being.

Requires at least 2 years in the field with supervision experience; a High School diploma or GED is required, bachelor's degree preferred. NYC driving eligibility and certifications such as AMAP, CPR/First Aid are expected.

Qualifications

• High School Diploma or GED required; bachelor's degree preferred.
• Minimum 2 years in the field with supervision experience.
• Valid NYC Driver’s License required and ability to transport individuals.
• Ability to complete and manage documentation and plan implementations.

Responsibilities

• Oversee DSP implementation of individuals’ plans aligned with services and goals.
• Plan and participate in community and program activities to meet goals.
• Promote choice, self-determination and inclusion for individuals.
• Engage individuals in skills development, socialization, and tasks.
• Provide supervision and train staff; ensure proper documentation.
