Job summary

HeartShare is hiring a Sr Day Support Professional to ensure the implementation of person-centered services for each individual and to maintain their physical and emotional well-being. You will develop and review staff action plans, coordinate monthly schedules for community outings, and train staff on plans and documentation.

This is a full-time day shift in Brooklyn, NY with travel up to 25%. Initial compensation starts at an hourly base of $24.50 with potential bonuses and raises based on

Qualifications

• High School Diploma or GED required; bachelor’s degree preferred.
• Valid NYC Driver’s License required.
• Minimum 2 years in the field with supervision and paperwork.
• Minimum 2 years or more experience working with individuals with developmental disabilities.

Responsibilities

• Oversee DSP implementation of individuals’ plans across settings.
• Promote choice and self-determination for individuals.
• Engage individuals in skills development, socialization, and task participation.
