Job summary

HeartShare seeks a Sr Day Support Professional to ensure the implementation of person-centered services for individuals, including oversight of plans, goals, and outcomes. This role focuses on promoting choice, self-determination, and active participation in community activities.

Responsibilities include coordinating monthly schedules, supervising DSPs, and maintaining thorough documentation. Requires at least 2 years in the field, a NYC license, and relevant certifications (AMAP, CPR/First Aid,

Qualifications

• High school diploma or GED required; bachelor’s degree preferred.
• Minimum 2 years of field experience with individuals with developmental disabilities.
• Valid NYC Driver’s License required.
• AMAP, CPR/First Aid and SCIP certifications maintained as required.

Responsibilities

• Oversee DSP implementation of individuals’ plans and outcomes.
• Plan and participate in community outings and in-house activities.
• Promote self-determination and individual goals in all services.
• Engage individuals in skills development, socialization, and daily routines.
• Ensure documentation, training, and plan dissemination are completed promptly.
