Job summary

Heartshare Wellness LTD invites applications for a Day Support Professional to provide person-centered services for program participants, ensuring safety and well-being in both center-based and community settings. You will help implement individualized goals, support social participation, and assist with transportation and supervision as required.

Qualifications

• High school diploma or GED required.
• NYS driver’s license preferred.
• CNA or Direct Support Professional (DSP) certificate preferred.
• One+ years' experience with individuals with developmental disabilities preferred.

Responsibilities

• Implement individual program participant's Day Services Plan, aligning activities with goals and outcomes.
• Train participants according to goals, facilitate skills development and community participation.
• Support participants in forming friendships and engaging with community members.
• Transport participants in agency vehicles for community inclusion and home commutes.
• Participate in group trips and interdisciplinary team meetings as needed.
• Maintain accurate behavior, goal, and incident data; ensure timely documentation.
• Uphold participant safety and supervision at all times.
• Maintain group room organization, cleanliness, and reporting of hazards.
• Pursue ongoing training and maintain CPR/First Aid and SCIP certifications.

Education

High school diploma or GED
