Job summary

Heartshare in New York is seeking a Day Support Professional to implement person-centered Day Services Plans for program participants, guiding activities at the center and in the community while safeguarding participant safety and well-being.

Responsibilities include training participants, facilitating inclusion, transporting participants in agency vehicles, assisting with documentation, and supporting group trips; strong communication and compassion are essential.

Qualifications

• High school diploma or GED required.
• CNA or DSP certificate preferred.
• One or more years' experience working with individuals with developmental disabilities preferred.
• Valid NYS driver's license preferred but not required.

Responsibilities

• Implement Day Services Plan for each participant, aligning activities with individual goals.
• Train participants to develop skills, socialize, and participate in group and community activities.
• Facilitate community inclusion and support relationships with neighbors, coworkers, and peers.
• Transport participants in agency vehicles for community activities, services and home.
• Complete written reports and documentation in a timely manner and assist in data collection.

Skills

CNA/DSP preferred
