Job summary

Heartshare is seeking a Day Support Professional in New York to implement person-centered Day Services Plans for program participants, both at the center and in the community. The role focuses on achieving individual goals, ensuring safety and well-being, and facilitating active participation in daily activities.

Responsibilities include training participants, promoting social engagement, transporting participants as needed, and completing timely documentation.

Qualifications

• High school diploma or GED required.
• Valid NYS driver’s license required.
• CNA or DSP certificate preferred.
• One+ years’ experience with individuals with developmental disabilities preferred.

Responsibilities

• Implement Day Services Plan to achieve individual goals in center and community activities.
• Train participants to develop skills and social participation.
• Facilitate community inclusion and friendships with others.
• Transport participants in agency vehicles for activities and home.
• Complete timely written reports and documentation.
• Ensure participant safety and supervise group activities.
• Provide crisis intervention and counseling as required.
• Report incidents to program management immediately.
