UCI Health in Irvine, CA is seeking a clinical nurse to collaborate with interdisciplinary teams to develop and implement a plan of care for a specific patient group. The role ensures care coordination across disciplines and supports staff throughout the care continuum, including evaluating outcomes and readiness for discharge.

It also serves as a mentor to students and new nurses, acting as a PCC when requested by leadership.

Formación

• Two years of recent nursing experience in a Critical Care, Emergency Department, or Perioperative setting.
• PALS certification upon hire (from the ad)
• ACLS certification upon hire (from the ad)
• Must demonstrate excellent customer service skills
• Ability to work variable shifts including evenings, nights, weekends, and holidays
• Ability to read, write and communicate effectively in English

Responsabilidades

• Collaborate with interdisciplinary teams to develop and implement a plan of care for a specific group of assigned patients.
• Coordinate care between disciplines and support staff throughout the continuum of care.
• Evaluate patient outcomes and readiness for discharge and revise the plan as necessary.
• Serve as a role model and mentor to students and nurses new to the unit.
• Function as a Patient Care Coordinator (PCC) as requested by the supervisor/manager
