UCI Health in California is seeking an experienced clinical nurse to collaborate with interdisciplinary teams, develop and implement comprehensive care plans, and coordinate care across disciplines. The role includes evaluating outcomes, guiding discharge readiness, and mentoring new staff as a Patient Care Coordinator upon request.

The position requires a BSN, 1 year of critical care and ICU experience, and flexibility to work evenings, nights, weekends, and holidays.

Qualifications

• Minimum 1 year recent critical care experience.
• Minimum 1 year recent ICU nursing experience or completion of 6 month UC Irvine Medical Center ICU orientation.
• All external candidates must have a Bachelor of Science in Nursing.
• Ability to work variable shifts including evenings, nights, weekends, and holidays.
• Ability to read, write and communicate effectively in English.
• Ability to maintain flexibility and work well in a fast paced, constantly changing environment.
• Ability to maintain a work pace appropriate to the workload.

Responsabilités

• Collaborate with interdisciplinary teams to develop and implement a plan of care for a specific group of assigned patients, ensuring coordination of care between other disciplines and support staff.
• Evaluate patient outcomes, effectiveness of the plan of care, and readiness for discharge; revise plan or make referrals as necessary.
• Serve as a role model and mentor to students and nurses new to the unit; act as a PCC (Patient Care Coordinator) as requested by supervisor/manager.
