Qualificações

• Bachelor’s degree in social work, psychology, counseling, human services, or related field.
• Minimum of three (3) years of professional experience in case management, care coordination, behavioral health, crisis intervention, or community-based human services.
• Minimum of one (1) year of lead-level support experience (coaching, training, mentoring, workflow coordination).
• Experience coordinating referrals and connecting clients to behavioral health, medical, social services, benefits, and community resources.

Responsabilidades

• Provide lead-level support and guidance to staff on client needs, care coordination, referrals, documentation, and follow-up.
• Coordinate crisis follow-up and case management services across the continuum.
• Assist in ensuring timely connections to behavioral health, medical, social service, and community resources.
• Promote client engagement, barrier removal, and continuity of care across transitions from crisis to outpatient services.
• Document case management activities in accordance with policies and ensure quality standards.
