Ada S McKinley Community Services Inc in Chicago, IL is seeking a Crisis Continuum of Care Lead Case Manager to provide lead-level case management, coordination, and crisis follow-up for children, adolescents, adults and families.

This role focuses on timely follow-up, engagement, referrals, linkages to care, and continuity of services across the crisis continuum, while mentoring staff and coordinating resources.

Qualifications

• Bachelor’s degree in psychology or related field required.
• Minimum of 3 years in case management, care coordination, behavioral health, crisis intervention, or community-based human services.
• Minimum of 1 year leading staff, coaching, training, case consultation, mentoring, or workflow coordination.

Responsibilities

• Provide lead-level support, consultation, and guidance to staff on client needs, care coordination, referrals, documentation, and follow-up.
• Coordinate crisis follow-up and case management services across the Crisis Continuum of Care.
• Assist in ensuring timely connections to behavioral health, medical, social services, and community resources.
• Support transitions from crisis intervention to outpatient and community-based services.
• Monitor follow-up requirements and assist with referrals and linkages within set timelines.
• Promote client-centered, recovery-oriented practices in care coordination.
