Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or related field is required.
• Minimum 3 years in case management, care coordination, behavioral health, crisis intervention, or community-based human services.
• Minimum 1 year of lead-level support including staff coaching, training, case consultation or mentoring.
• Experience coordinating referrals and connecting clients to behavioral health, medical, social services, benefits, and community resources.

Responsibilities

• Provide lead-level support, consultation, and guidance to case management staff on client needs, referrals, documentation, and follow-up.
• Coordinate crisis follow-up and case management services across the continuum of care.
• Assist in ensuring timely connections to behavioral health, medical, social services, and community resources.
• Support transitions from crisis intervention to outpatient and community-based services.
• Develop and maintain collaborative relationships with partners to improve access to services.
