Ada S. McKinley Community Services Inc. is seeking a Crisis Continuum of Care Case Manager to coordinate care across Mobile Response and Stabilization Services (MRSS), Crisis Stabilization, and the 590 Crisis Program.

You will develop individualized service plans, connect clients with behavioral health and community resources, and document in line with Medicaid and DHS requirements. The role requires a Bachelor’s degree in a related field, registration as Mental Health Professional (MHP) per

• Must qualify as a Mental Health Professional (MHP) under Illinois DHS guidelines.

Responsibilities

• Provide care coordination and stabilization services across MRSS, Crisis Stabilization Services, and the 590 Crisis Program.
• Conduct post-crisis follow-up and engagement to promote stabilization and reduce repeat crisis utilization.
• Coordinate referrals to outpatient behavioral health, psychiatry, primary care, housing, benefits, peer support, substance use treatment, and community resources.
