Job summary

BestCare Treatment Services, Inc. is seeking a dedicated Case Manager to coordinate care, manage data, and provide direct clinical services within our crisis stabilization program.

The role focuses on building strong client and provider relationships, tracking program outcomes, and supporting recovery goals. The position works with a multidisciplinary team to connect clients to benefits, transportation, and primary care, while ensuring timely documentation and compliance to promote crisis

Qualifications

• Bachelor’s degree in behavioral science required (Level II) or Associate degree plus 4 years in social services (Level I)
• Experience working with individuals with severe and persistent mental illness (SPMI)
• QMHA-I certification or eligibility to register for QMHA-R within one week of hire and certification within six months
• Must maintain a valid Oregon Driver License and be insurable under agency policy

Responsibilities

• Collaborate with staff as a treatment team member on the Mental Health Team
• Assist clients to access resources, benefits, and entitlement programs
• Support clients in achieving their ISSP/Individual Services & Support Plan goals
• Develop and maintain relationships with community partners (e.g., Self-Sufficiency, Seniors/People with Disabilities, Public Health, primary care clinics)
• Assist clients in ACT-Assertive Community Treatment with Personal Recovery Plan
• Maintain clinical documentation in the EHR within BestCare timelines and regulatory standards
