Job summary

Greenbrook Medical is building a new standard in primary care for seniors in the Greater Tampa Bay Area. As Complex Care Manager, you will coordinate high-risk patients across hospitals, SNFs, and clinics to ensure proactive, seamless care from admission to recovery in the community.

You will work with physicians, center managers, and care teams to prevent unnecessary hospital utilization, drive timely follow-ups, and improve patient outcomes. Strong data skills and bilingual Spanish are a plus.

Qualifications

• Active RN or LPN license.
• 3+ years of nursing, care management, or complex care coordination experience.
• Experience in managed care, Medicare Advantage, or value-based primary care.
• Strong clinical judgment and patient triage skills.
• Experience coordinating care across hospitals, SNFs, specialists, and outpatient settings.
• Strong Excel skills and comfort using data to prioritize work.

Responsibilities

• Own the clinical coordination of Greenbrook's most medically complex patients across hospital admissions through recovery in the community.
• Coordinate with hospitals, SNFs, specialists, and clinic teams to ensure seamless, proactive care.
• Prevent avoidable ER visits, reduce SNF utilization, and ensure timely transitions of care.
