Job summary

Greenbrook Medical is reshaping senior primary care in the Tampa Bay area. You will manage a patient panel of ~500 Medicare Advantage patients, focusing on prevention, chronic disease management, and healthy aging. The role emphasizes reduced administrative burden through dedicated coding support and coordinated care with a skilled team.

Expect 30-45 minute visits, 9:00 AM to 3:30 PM schedules, and a strong emphasis on outcomes, patient experience, and collaboration in a growing organization.

Qualifications

• Board certified or board eligible in internal medicine, family medicine, geriatrics, or related primary care specialty.
• Active Florida medical license or ability to obtain one.
• Experience in primary care, managed care, value-based care, or residency in internal medicine or family medicine.
• Passion for building lasting relationships with patients.
• Collaborative, humble, team-first mindset.

Responsibilities

• Deliver comprehensive primary care for a panel of ~500 Medicare Advantage patients.
• Coordinate care with multidisciplinary team to ensure seamless transitions.
• Identify and close care gaps to improve quality metrics.
• Leverage Epic or eClinicalWorks and population health data to guide care.
• Provide exceptional patient experiences with empathy and education.
• Contribute ideas to improve clinical workflows and patient outcomes.
