Job summary

Greenbrook Medical is seeking an Acute Care & Transitions Manager (RN or LPN) to own the day‑to‑day coordination of patients moving through acute care settings and SNF stays. You will collaborate with hospital partners and Greenbrook teams to ensure safe, timely transitions back to the community.

You’ll monitor admissions, manage discharge planning, and provide clinical support to avoid unnecessary ED utilization. Travel across the Greater Tampa Bay Area is required.

Qualifications

• Active RN or LPN license.
• 3+ years nursing, acute care, care management, or transitions of care experience.
• Experience with hospitals, EDs, SNFs, or inpatient care teams.
• Strong clinical judgment and patient triage skills.
• Spanish-speaking能力 a plus.

Responsibilities

• Own Acute Care & Hospital Transitions: monitor admissions, coordinate discharge plans, ensure follow‑up.
• Coordinate SNF Transitions: track SNF progress, ensure timely transitions back to clinic.
• Build Strong Acute Care Partnerships: establish relationships with hospitalists and SNF teams.
• Represent Greenbrook as a clinical partner and improve discharge workflows.
• Drive accountability for transitions: monitor outcomes and optimize processes.
