Job summary

Greenbrook Medical in the Tampa Bay area seeks an experienced Complex Care Manager to coordinate care for our most medically complex patients across hospitals, SNFs, and clinics. You’ll drive proactive, high-touch care from admission through recovery and ensure timely follow-ups.

As a licensed RN or LPN with 3+ years in care coordination, you’ll partner with physicians, pharmacists, and care teams to reduce avoidable ER visits, improve transitions, and deliver better outcomes for seniors.

Qualifications

• Active RN or LPN license.
• 3+ years of nursing or care coordination.
• Experience in managed care or Medicare Advantage.
• Strong clinical judgment and patient triage.
• Experience coordinating care across hospitals, SNFs, and outpatient settings.
• Bilingual in Spanish is a plus.

Responsibilities

• Own the care of Greenbrook's highest-risk patients across the full continuum.
• Monitor hospital and SNF admissions daily and proactively manage transitions back into primary care.
• Ensure patients receive timely follow-up appointments, medication reconciliation, and ongoing care planning.
• Serve as a clinical resource for patient triage and urgent needs.
• Collaborate closely with physicians, center managers, pharmacists, hospitalists, SNFs, and community partners.
