Job summary

Greenbrook Medical in Tampa Bay is seeking an Acute Care & Transitions Manager (RN or LPN) to own day-to-day coordination of patients through acute care settings and SNF transitions. You will monitor hospital admissions, coordinate discharge plans, and ensure timely post-discharge follow-up while collaborating with hospital and clinic teams.

Qualifications

• Active RN or LPN license is required.
• 3+ years of nursing, acute care, care management, or transitions of care experience.
• Experience working with hospitals, SNFs, or inpatient care teams.
• Strong clinical judgment and patient triage skills.
• Experience with transitions of care, discharge planning, or post-acute care coordination.
• Willingness to travel across the market.

Responsibilities

• Own Acute Care & Hospital Transitions: monitor admissions, coordinate discharge planning, ensure follow-up.
• Coordinate SNF Transitions: monitor SNF admissions, coordinate transitions back to clinic.
• Build strong partnerships with hospitalists, SNF teams, and Specialists.
• Drive accountability for transitions: use data to identify trends and improve workflows.
