Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to lead care for medically complex seniors in community settings across Multnomah & Clackamas Counties, OR. You will manage a patient panel with a comprehensive approach, coordinating with PCPs and facilities to improve outcomes and reduce hospitalizations.

This full-time, field-based role offers flexible scheduling, strong interdisciplinary support, and long-term patient relationships.

Qualifications

• Graduate of an accredited MSN or DNP program (NP) or accredited PA program.
• Active and unrestricted NP or PA license or ability to obtain by start date.
• AANP/ANCC or NCCPA certification depending on role.

Responsibilities

• Conduct comprehensive patient assessments and develop care plans.
• Manage acute and chronic conditions with diagnostics/interventions.
• Lead complex, individualized treatment plans and goals-of-care discussions.
• Collaborate with interdisciplinary team and caregivers; provide education.
