Job summary

Qualifications

• BLS certification required within first 90 days.

Responsibilities

• Provide care for patients with acute and chronic illnesses, especially older adults.
• Lead a care team and coordinate care across settings.
• Engage hospitalists for hospitalized patients regardless of affiliation.
• Recruit patients and providers; mentor PCPs and supervise daily huddles.
• Lead weekly huddles and support leadership as needed.
