Job summary

ChenMed LLC is seeking a Primary Care Physician (MD/DO) to provide comprehensive care for an older adult patient panel. The physician will lead a care team, coordinate with hospitalists, and deliver outcomes-driven, value-based care.

Requires state medical license, DEA, and BLS within 90 days. Board certification preferred; willingness to travel locally for training and recruitment. English fluency is essential.

Qualifications

• Fluency in English, verbal and written.
• Ability and willingness to travel locally for training and recruiting.
• MD or DO in Internal Medicine, Family Medicine, Geriatrics or related specialty is required.
• Must have or be able to obtain a State Medical License.
• Board certification preferred; board eligibility required.
• Current DEA number for schedule II–V substances.
• BLS certification within first 90 days of employment.

Responsibilities

• Provide care for older adults with acute and chronic illnesses.
• Lead and collaborate with the care team (medical assistant, care facilitator, care coordinator).
• Engage hospitalists and other specialists when needed to coordinate patient care.
• Ensure documentation, education, and health promotion for patients.
• Support center leadership and participate in risk and quality programs.
