Job summary

Qualifications

• Bachelor’s degree in a relevant field and at least 1 year experience in human services preferred.

• Familiarity with documentation and reporting requirements for care programs.

Responsibilities

• Carries a caseload and develops person-centered service plans with residents.
• Coordinates with treatment teams and community partners to support recovery.
• Documents interactions, assessments, and progress in a timely, compliant manner.
• Assists residents with activities of daily living and crisis de-escalation as needed.
