Job summary

The Jewish Board of Family and Children's Services seeks a Case Manager to lead direct care within its supportive housing programs in New York. Qualifications

Responsibilities

  • Carries a caseload and creates person-centered service plans with residents.
  • Works with the most complex clients and oversees case staff.
  • Ensures plans and progress notes are completed per regulatory standards.
  • Links residents to community-based treatment and coordinates services.
  • Participates in intake, assessments, discharge, and high-risk case meetings.
  • Assists with crisis de-escalation and safety planning as needed.
