Job summary

The Jewish Board is seeking an experienced Case Associate to support adults with serious mental illness in a supported housing program in New York. You will manage resident caseloads, document care interactions, and coordinate with health and social service partners to promote independence and recovery.

Responsibilities include direct resident support, administrative tasks, and participation in meetings. A strong emphasis on person-centered planning, crisis de-escalation, and anti-racist

Qualifications

• AA degree with two years of community mental health experience required
• HS/HSE with at least five years of community mental health experience required
• Familiarity with Microsoft Office suite (Excel, Word, PowerPoint)

Responsibilities

• Provide resident support within program regulations and agency mission.
• Document resident interactions and progress timely for billing and care planning.
• Conduct monthly unit/apartment inspections and assist with moving or transport as needed.
• Coordinate with other services and advocate for residents with external providers.
• Facilitate resident meetings and support person-centered planning.
• Travel to visit residents in apartments and escort them when required.
