Job summary

ACMH seeks a Senior Case Manager/Case Manager for the Bronx Treatment Apartment to coordinate care and deliver community rehabilitation services.

Responsibilities include home visits, psychosocial assessments, liaison with psychiatric care, medication monitoring, and developing individualized service plans with the treatment team. The role involves housing support and crisis intervention as needed.

Qualifications

• Skills in case coordination and service planning.
• Strong written and verbal communication abilities.
• Experience documenting client progress and plans.

Responsibilities

• Conduct home visits for assigned caseload.
• Obtain psychosocial histories and assess functional skills.
• Coordinate with psychiatric care providers for access to services.
• Monitor medication adherence and side effects; track health indicators.
• Develop individualized Service/Support Plans with the treatment team.
• Deliver housing and community reintegration services as needed.
• Document progress and update plans regularly.
• Provide crisis intervention and safety training as required.
