Responsibilities

  • Manage a caseload and develop person-centered service plans with residents.
  • Coordinate with residents and multidisciplinary teams to meet goals and ensure regulatory compliance.
  • Document interactions, assessments, plans, and progress in a timely manner for billing and reporting.
  • Provide advocacy and support to residents, linking to community resources and treatment services.
  • Support residents in medication management and crisis de-escalation when needed.
  • Assist with intake, assessments, and discharge planning; participate in staff meetings and trainings.
