Job summary

The Jewish Board seeks a Case Associate to support adults with mental illness in supported housing programs. You will aid residents to live independently, advocate for services, and assist with daily living tasks while maintaining case documentation and regulatory compliance.

Requirements include relevant education/experience in community mental health, strong communication, and crisis de-escalation abilities. Travel to community visits may be required.

Qualifications

• AA degree and two years of experience in a community mental health setting or
• HS/HSE degree and at least five (5) years of experience in a community mental health setting.

Responsibilities

• Carry a caseload of residents and provide services within program regulations and agency mission.
• Document all resident interactions and ensure monthly billing documentation requirements where applicable.
• Assist residents with daily activities, medication management, and attending appointments.
• Facilitate monthly roommate meetings and participate in regular staff meetings and trainings.
