Job summary

Qualifications

  • Ability to travel to meet residents and attend trainings.

Responsibilities

  • Carry caseload and develop recovery-focused service plans with residents.
  • Manage complex caseload and ensure plans are signed and updated.
  • Document resident interactions, assessments, and progress for records and billing.
  • Coordinate with community services and accompany residents to appointments as needed.
  • Provide safety planning, crisis de-escalation, and advocacy for residents.
