Accessible Space, Inc. (Line Drive Apartments) in Reno, NV, is seeking a dedicated Full-Time Case Manager to support residents in achieving goals and quality of life improvements.

The role involves admissions, care coordination, and ongoing funding communication within a supportive PSH community. The position emphasizes collaboration with clinical teams, families, and funding sources, ensuring accurate documentation and patient-centered planning.

Qualifications

• LSW credential or a Master’s Degree in social work or closely related field.
• A minimum of three years of experience as a case manager in rehabilitation, healthcare, or social services.
• Experience in rehabilitation programs, day treatment, or residential settings for individuals with traumatic brain injuries.
• Ability to collaborate effectively within a multidisciplinary team.
• Excellent written and oral communication skills with diverse populations.
• Knowledge of disability advocacy groups, accessible housing, and long-term options for adults with TBIs. Proficiency with MS Office Suite and standard business apps.

Responsibilities

• Conduct admissions and orientation, including tours, rights and responsibilities, schedules, and expectations.
• Coordinate interdisciplinary assessments and support development and review of treatment plans.
• Lead conferences with patients, families, clinical staff, and funding sources during initial, monthly, and discharge meetings.
• Facilitate ongoing funding authorization and communicate with insurers and funding sources.
• Lead clinical treatment planning sessions with the clinical team.
• Maintain accurate, confidential clinical documentation of progress and participation.
• Serve as a point of contact for families and caregivers, providing updates and discharge planning support.
