Job summary

Southwest Behavioral & Health Services in Prescott Valley is seeking a dedicated Case Manager for Supported Housing to help residents achieve stable, independent living. You will coordinate intakes, lease processes and housing services, guided by recovery principles and person-centered care.

Responsibilities include housing compliance, landlord liaison, and crisis support coordination, with emphasis on communication, organization and problem solving to serve diverse communities in need.

Qualifications

• Bachelor’s degree in a related field with relevant experience.
• Experience working with underserved or vulnerable populations preferred.
• Ability to drive and pass background checks.

Responsibilities

• Perform intakes, screenings and assessments for residents.
• Manage leases, addendums and monitor lease compliance.
• Coordinate housing procedures per HUD regulations and Fair Housing Act.
