Advanced Home Health and Hospice in Reno, NV seeks a Home Health RN Case Manager to coordinate care, perform skilled nursing tasks, and implement individualized care plans in patients' homes.

The role requires independent judgment, strong clinical skills, and excellent communication with patients, families, and a multidisciplinary team; schedule is full-time, M-F, 8-hour shifts, with mileage reimbursement.

Qualifications

• RN license (state of practice) is required.
• Minimum 2+ years of nursing experience; home health experience preferred.
• Current CPR certification.
• Knowledge of OASIS documentation system preferred.
• Strong clinical assessment and communication skills.

Responsabilités

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care, including wound care and medication administration.
• Coordinate and supervise home health aides and other professionals.
• Communicate with patients, families, and the care team about progress and plans.
• Ensure regulatory compliance and accurate documentation.
• Evaluate patient outcomes and adjust care plans as needed.
• Participate in quality improvement and continuing education.
