The Servant Center is seeking a dedicated case management professional to support clients across medical, behavioral, and housing needs. You will assess, plan, and coordinate services, while advocating for clients and providing counseling support in a residential setting.

Responsibilities include intake, documentation in HMIS, and coordinating housing transitions with entry and housing staff. A valid NC driver’s license, CPR/First Aid, and experience with vulnerable populations are required.

Formación

• Minimum of a year of case management experience.
• Experience with homeless or vulnerable populations preferred.
• CPR/First Aid certification is required.
• Valid North Carolina driver's license and ability to be added to the agency's insurance policy.

Responsabilidades

• Conduct needs assessments (medical, behavioral, housing) and plan individualized care.
• Coordinate services (employment, SSI/SSDI, VA, Medicaid, food stamps, healthcare) and transport as needed.
• Provide advocacy, crisis intervention, and supportive counseling.
• Develop housing plans and coordinate with Coordinated Entry and housing staff.
• Welcome and orient clients, provide daily check-ins, and support adjustment.
• Complete intake forms, maintain case notes, and HMIS/database entry.
• Collaborate with staff, social workers, and community partners.
