• Develops with each client a detailed Service Plan centered on housing stability and actionable steps.
• Maintains timely case notes in internal documentation systems.
• Provides two case management sessions per month to participants.
• Monitors progress toward goals and coordinates service linkage to housing resources.
• Gathers data and reports statistics to the Director as required.
• Develops transitional plans 90 days prior to discharge to ensure ongoing support.
• Advocates for clients to reduce systemic barriers to success.
• Collaborates with internal and external stakeholders for coordinated service delivery.
