Job summary

VOALA is seeking a Senior Case Manager II in the Los Angeles area to mentor the case management team and coordinate services for clients. The role blends administrative duties with field outreach to ensure service plans meet client needs.

Responsibilities include assessing client needs, educating on resources, developing plans, scheduling follow-ups, and ensuring progress. The position emphasizes supporting vulnerable populations with dedicated teamwork.

Responsibilities

• Conducts comprehensive screenings and assessments to develop a case plan.
• Develops support systems, coordinates move-in activities, rental negotiations and referrals.
• May make home and field visits.
• Monitors case managers to determine quality and effectiveness of services.
• Reviews cases with Program Coordinator and Case Management Team.
• Audits case files for completeness and accuracy.
• Conducts crisis intervention as necessary.
• Identifies and coordinates services for veterans and families to maintain self-sufficiency.
• Makes referrals to VA/community providers for resources.
• Provides counseling and advocacy support to clients and families.
• Maintains and secures program assets and client records.
• Develops and assists with workshops and participates in outreach events.
• Other duties as assigned.
