Job summary

Jewish Community Services of South Florida (JCS) is seeking a Case Manager to engage with and assist elderly adults, providing intake, assessment, case management, referrals, education, and ongoing support.

The role emphasizes developing comprehensive service plans, coordinating with families, and utilizing community resources to support emotional and physical well-being. Thorough training and licensing in social work are preferred; bilingual abilities are valued.

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• Minimum 2 years of relevant experience.
• LCSW, LMFT, or LMHC license preferred or license eligible.
• Proficiency with Microsoft Office and web-based client data systems.

Responsibilities

• Complete assessments and program documents approved by Department Director.
• Monthly in-person home visits and phone check-ins.
• Development of Case Management Treatment Service plan.
• Crisis intervention and information/referrals to resources.
• Arrange for client services as needed and coordinate care.
• Participate in trainings, supervision, and meetings.
• Data entry into a web-based system.
• Work performed through home visits, Kendall Office, and HQ as requested.
