The ideal candidate holds a social work-related degree with 2+ years in case management, strong documentation skills, and proficiency with MS Office. JCS champions compassionate service and community empowerment.

Qualifications

• Bachelor's degree in Social Work or related field required; Master's preferred.
• Minimum 2+ years of case management or related human services experience.
• Knowledge of community resources and ability to connect clients with supports.
• Experience conducting assessments and developing service plans with documentation.
• Proficiency with Microsoft Office and electronic documentation systems.

Responsibilities

• Provide individualized case management per agency and grant requirements.
• Conduct client assessments and develop service and review plans.
• Establish and maintain advocacy relationships with clients.
• Monitor progress, evaluate outcomes, and determine goal achievement.
• Coordinate services with community organizations and service providers.
• Maintain accurate documentation per agency policies and funding requirements.
• Participate in TPM activities and contribute to program development.
