Job summary

Jewish Community Services of South Florida (JCS) is hiring a Case Manager to engage with and assist elderly adults, providing intake, assessment, case management, referrals, and education. The role focuses on building ongoing connections to ensure emotional and physical well-being through comprehensive service plans.

Responsibilities include home visits, documentation, crisis intervention, and collaboration with families.

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• Minimum 2 years of relevant experience preferred.
• LCSW/LMFT/LMHC license preferred or license eligible.
• Ability to speak fluent Spanish, Yiddish, or Russian is a plus.
• Strong computer skills including Word, Excel, PowerPoint, and Outlook.

Responsibilities

• Complete assessments and program documents approved by the Department Director.
• Monthly in-person home visits and phone check-ins as required.
• Develop Case Management Treatment Service plan.
• Crisis intervention.
• Provide information and referrals to community resources.
• Arrange for client services as needed.
• Participate in trainings, supervision, and meetings (in person/Zoom).
• Case coordination and monitoring.
• Provide direct support to clients.
• Data entry into a web-based client data system.
• Work through home visits, in Kendall and HQ offices as requested.
