The Jewish Board is seeking a Case Manager for its Adult Residential Division in New York City. This senior direct-care role guides residents toward independence, using a harm-reduction and culturally competent approach.

Responsibilities include managing complex caseloads, coordinating services, and leading collaborative planning with residents while ensuring regulatory-compliant plans and progress notes. Strong communication and crisis-management skills are essential.

Qualifications

• Bachelor's degree in arts or sciences; 1+ year experience in human services preferred.
• Experience working with individuals living with serious mental illness, substance use disorders, or trauma.
• Ability to travel to visit residents and escort residents in the community.

Responsabilités

• Carries a caseload and engages residents in person-centered, collaborative service planning.
• Manages the most complex cases and maintains high caseload standards.
• Ensures service plans are completed and signed by residents per regulatory guidelines.
• Documents all resident interactions, assessments, and progress timely.
• Links residents to community-based treatment and coordinates services with providers.
• Participates in intake, assessments, discharge, and high-risk case meetings.
