Job summary

ACMH, Inc. in Queens, NY is seeking a Senior Case Manager/Case Manager to coordinate community rehabilitation services and housing support for adults with serious mental illness.

The role reports to the Program Director and operates on a Tue-Sat schedule (10:00am-6:00pm) across Queens and surrounding areas. The position requires a Master’s in psychology or related field (BA with 3+ years acceptable) and bilingual English/Mandarin.

Qualifications

• Master’s degree preferred; BA with 3+ years of direct care experience acceptable.
• Bilingual English/Mandarin required.

Responsibilities

• Conducts home visits for assigned caseload.
• Obtains psychosocial histories and conducts functional skills assessments.
• Liaisons with psychiatric care providers; ensures access to services and entitlements.
• Monitors medications and side-effects; administers health screening tools.
• Develops and updates individualized Service/Support Plans with the treatment team.
• Delivers housing support and community rehabilitation services as needed.
• Maintains client charts, documents progress, and completes incident reports.
• Provides crisis intervention and staff coverage as needed.
