Job summary

ACMH is seeking a Senior Case Manager/Case Manager for Sabra Goldman House in Queens Village, NY. This role provides case coordination and delivers community rehabilitation services under the supervision of the Program Director.

You will conduct home visits, assess psychosocial histories, and coordinate psychiatric and medical services. The position requires scheduling Tue-Sat or Sun-Thu from 11:00am–7:00pm and involves maintaining charts and Service/Support Plans.

Qualifications

• Strong written and verbal communication skills.
• Experience in case management or related human services fields.
• Experience working with adults with serious mental illness preferred.

Responsibilities

• Conducts home visits and manages an assigned case load.
• Obtains psychosocial histories and conducts functional skills assessments.
• Establishes and maintains liaison with primary provider of psychiatric care.
• Assures access to psychiatric, substance use, medical services, and entitlement benefits.
• Monitors client medication, including adherence and side-effects.
• Administers standardized health screening tools.
• Develops an individualized rehabilitation plan (Service/Support Plan) in consultation with the treatment team and client.
