Job summary

ACMH, INC. is seeking a Senior Case Manager/Case Manager in Queens Village. The role focuses on case coordination, community rehabilitation services, crisis intervention, and housing supports, reporting to the Program Director.

Typical schedule is Monday through Friday, 8:00am-4:00pm, with responsibilities spanning home visits, care coordination, and team collaboration. Candidates should hold an MA or BA in psychology or a related field, with experience in mental health, strong communication

Qualifications

• M.A. in psychology or a human service related field is required.
• B.A. in psychology or a human service related field with at least 3 years of direct care experience.
• Strong written and verbal communication skills; Master’s degree preferred.

Responsibilities

• Conduct home visits for assigned caseload.
• Obtain psychosocial histories and assess functional skills.
• Coordinate with psychiatric care providers and ensure access to services.
• Monitor medications, adherence, and side effects.
• Develop individualized rehabilitation plans with the treatment team.
• Deliver community rehabilitation and housing support services.
• Document responses and update the Service/Support Plan.
