Job summary

ACMH in Queens, NY is seeking a Senior Case Manager to coordinate case management and deliver community rehabilitation services. This role requires a Master or BA with relevant direct-care experience and fluency in English and Mandarin.

Schedule is Tue–Sat, 10:00am–6:00pm, with housing and entitlements support. The candidate will conduct home visits, psychosocial assessments, and liaise with psychiatric providers; manage medications and health screenings; and develop individualized

Qualifications

• M.A. in psychology or a human service related field is required.
• B.A. in psychology or a human service related field with at least 3 years of direct care experience is required.
• Master’s Degree preferred.
• Bilingual English/Mandarin required.

Responsibilities

• Conducts home visits for assigned caseload.
• Obtains psychosocial histories and conducts functional skills assessments.
• Maintains liaison with psychiatric care providers and ensures access to services and entitlements.
• Monitors medication adherence and side-effects; administers health screenings.
• Develops individualized rehabilitation plans with input from the treatment team.
• Delivers community rehabilitation and housing support services.
• Documents client progress and updates plans; trains clients in safety and procedures.
• Provides coverage as needed for other staff and acts as Qualified Mental Health Staff Person.
