Job summary

ArchWell Health is seeking a Social Work Coordinator to support the social, emotional, and practical needs of members by linking them to community resources and assisting with social services to reduce barriers to health and well-being. You will work closely with Social Workers, providers, and care teams to navigate resources, complete forms, and address Social Determinants of Health.

Qualifications

• High School Diploma or GED required.
• Bachelor’s or Master’s degree in Social Work or related field preferred.
• Minimum 1 year of experience in clinical administrative support, social services, care coordination support, community outreach, healthcare administration, or related role.

Responsibilities

• Outreach calls to assess needs related to Social Determinants of Health (SDOH).
• Connect members with community resources and services (food, housing, transportation, etc.).
• Assist LMSW/LCSW with care coordination, discharge planning, and higher levels of care placement.
• Help members navigate benefits and community programs.
• Serve as resource guide for members and families seeking social/financial/environmental support.
